Requirements for student visa

Visa Application Form

1. Visa Application Form fully completed in English-Triplicate (FA form No: 2)
2. Passport size Color Photograph.
3. Original and valid Passport, Passport must be valid at least 6 months beyond the planned stay in the Philippines
4. Photocopy of page of passport with applicant's data

Air Ticket

1. Confirmed Air ticket/ one way and photocopy of confirmed air ticket.

Medical Report

A. Medical Certificate (FA Form No 11) signed by Medical Practitioner appointed by the Consulate of Rep. of the Philippines.
B. For Pathology Test and Medical Reports

C. Medical Report includes

• Blood Serology
• Urinalysis
• Stool Analysis
• Full Size Chest X-Ray

Police Clearance Report

1. Police Clearance Certificate duly authenticate by Notary public.

Affidavit of Financial Support

1. Affidavit of Financial Report alongwith latest Bank Statement not less than Five Lakhs.

Letter of Acceptance

1. Sealed Letter of Acceptance from respective University.

Reference in Philippines if any

1. Comprehensive List of Reference in Philippines alongwith detail addresses and telephone numbers.

Visa Fee

1. Student Visa Fee of Nrs, 28,400.00 to be deposited in the Standard Chartered Bank Nepal Limited on the Account of Consulate of the Rep. of the Philippines, Account No : 01-0209171-01
