Minor children below 15yrs. old traveling to the Philippines unaccompanied by or not joining a parent in the philippines

                                                                                                                                                                                                             
Children under fifteen (15) years of age going to the Philippines Traveling alone or unaccompanied by parents (or to be accompanied during travel by an individual other than a parent) or not coming to join a parent in the Philippines must comply with the following requirements:

  1. Duly accomplished visa application form (Form No. 2) in duplicate. A parent on behalf of the minor-applicant may accomplish this form.
  2. The minor's passport (the original passport itself must be submitted), which must be valid for at least six (6) months beyond the intended stay in the Philippines.
  3. Two 2" x 2" photographs of the minor taken within the last six (6) months.
  4. Affidavit of Request, Consent and Guaranty executed by either  or both parents, duly notarized by a local Notary Public or by a consular officer of Embassy or Consulate . This Affidavit must be presented before immigration officials at the port of entry in the Philippines.

Only upon presentation of the duly attested/notarized Affidavit will the Philippine Bureau of Immigration issue a Waiver of Exclusion Ground (W.E.G.) upon the minor's arrival in the Philippines.

  • Two photocopies of the Data Page of the passport of the : accompanying guardian; and the child
  • Original airline tickets together with photocopy.
