Requirements for renewal of Passport
- Request application letter for the passport renewal which is address to Consul General.
- Duly accomplished passport application form. Complete all the entries to avoid unnecessary delay. For minor applicant, a parent may accomplish the form on their behalf.
- Old passport and photocopy together with the pages showing latest Bureau of Immigration departure and arrival stamps.
- Submit proof of the applicant's actual residence, e.g. driver's license, state ID, employment ID, or other similar documents evidencing actual residence.
- Submit four photographs (size 1.77"x1.37") taken within the last six months. The photo must show the applicants front view, clad in decent attire and without eyeglasses. Digital photographs will not be accepted.
- For married women wishing to use their married name must submit the following documentary requirements:-
- For a marriage solemnized in the Philippines submit the original/certified true copy and a photocopy of marriage certificate.
- For a marriage solemnized outside the Philippines and celebrated within the Jurisdiction of the Philippines Consulate, Nepal, the following must be submitted:-
- Original and photocopy of Marriage Certificate.
- Duly accomplished Consulate Report of Marriage contracted abroad form.
- For minor, personal appearance of mother is required.
- Affidavit of Support, Consent & Guaranteed of either parent is required.
Payment of Fees:-
The fee for the renewal of passport Rs. 8,352/- is to be deposited in the Standard Chartered Bank.
