Requirement of cancellation of Philippine passport

• Request letter for cancellation stating that reason for cancellation which is address to Consul General.
• Affidavit form duly accomplished (Available at Philippine Consulate Office).
• Original passport with photocopy together with the pages showing Bureau of Immigration arrival & departure stamps.
• Fee amounting Rs………… is to be deposit in Standard Chartered Bank, any branch in the name of Consulate of the Rep. of the Philippines, A/C No. 01-0209171-01, as per detail below:
